Original Medicare generally does not cover routine non-emergency medical transportation in a car, sedan or wheelchair van. Medicare's ambulance benefit is different: it applies only in qualifying situations where ambulance transportation is medically necessary. Medicare Advantage, Medicaid and private-pay transportation must be evaluated under their own rules.
What is the difference between Medicare ambulance coverage and NEMT?
NEMT is planned transportation for a stable passenger traveling to an appointment, treatment, discharge destination or facility. An ambulance is an emergency or medically necessary medical transport service. Medicare describes ambulance coverage around situations where other transportation could endanger the patient's health, not ordinary rides to routine care.
What should you confirm with Original Medicare?
- Ask whether the requested service is being treated as ambulance transportation or routine NEMT.
- Confirm whether the passenger's condition and transportation need meet the plan's medical-necessity rules.
- Ask about the covered destination, provider requirements, documentation and possible cost sharing.
- Request a written explanation before the appointment when coverage is uncertain.
How are Medicare Advantage and Medicaid different?
Medicare Advantage plans can offer benefits that Original Medicare does not, but each plan sets its own covered destinations, ride limits, network rules and authorization process. Medicaid transportation is also governed by program and state rules. A member should confirm the current benefit with the plan or program rather than relying on a general internet answer.
When is private-pay NEMT a practical option?
Private pay can be useful when a family needs a specific vehicle, direct provider communication, a recurring schedule, a flexible route or a ride outside an insurance benefit. Ask for a written quote and describe the passenger's mobility before booking so the vehicle and assistance can be reviewed.
What should you ask before scheduling?
- Is the ride covered for this passenger, route and destination?
- Does the plan require prior authorization, a referral or a specific transportation broker?
- What vehicle and assistance level will be provided?
- Are waiting time, return rides, extra stops and cancellations covered?
- Who should the caregiver call if the appointment or discharge runs late?
When should you call 911?
Do not use NEMT as a substitute for emergency care. If the passenger has a sudden serious symptom, injury or change that needs immediate help, call 911. The transportation coverage question can be handled after the emergency has been addressed.
